General tab

• Form name — internal only; how the form appears in admin lists and on booking records
• Title — the heading shown above the form (e.g. "Book Your Ride")
• Subtitle — the line under it

Title and subtitle can be hidden entirely on the Settings tab (header visibility), and renamed per-language or per-campaign on the Labels tab — the General tab is the default text.
